  • past illnesses and injuries;
  • allergy data;

It is important that the 079U certificate has a record of     absence of contagious diseases. If there is no such entry,   take a certificate from the   leather dispensary certified by   seal   and the doctor’s signature;

  5. Results   analysis for eggs worm;

  6. Enterobiosis test results;

  7. Certificate of lack of contact with infectious patients (3 days before the shift).
