All documents can be submitted electronically by mail or in the office:

  • adult passport for registration of the contract;
  • completed application form for a child;
  • copy of the birth certificate or passport of the child (for the archive of the camp);
  • copy of the child's health insurance policy.

To camp departure:

  • a copy of the birth certificate or passport of the child (for settlement);
  • medical certificate 079-U (from a doctor at school or in the clinic);
  • medical certificate for the attendance of the swimming pool;
  • Sanitary-Epidemiological Medical Certificate (taken 3 days before departure).
Documents for participation in the camp program (provided at the time of departure):
  • ​Мед. справку – станд. формы для ребенка, отъезжающего в лагерь 79-У с указанием прививок
  • Справку об отсутствии контакта с инфекционными больными, в тч  COVID-19. Так же необходима отметка на справке об отсутствии педикулеза и чесотки (справка действительна 3 дня) 
  • Анализы на яйцеглист и энтеробиоз (можно указать в справке для бассейна. Эти анализы нужны, даже если ребенок не будет ходить в бассейн)
  • Копию св-ва о рождении ребенка (или паспорт, если исполнилось 14 лет) 
  • Копию медицинского страхового полиса  (ОМС)
  • Анкета на ребенка 
  • Заполненное заявление на санаторные процедуры
  • Согласие на обработку данных санаторием Малаховка  

Обращаем внимание, что все документы остаются в санатории по окончании смены.
