Bereznyaki is a completely new camp for children, located in the village of Lopukhinka in the Lomonosov district. The Bereznyaki Center is the rallying of the children’s team, civic and patriotic upbringing, support of family values, recreation and health improvement without interruption from the learning process, and concern for nature. The camp began its activities in the summer of 2018.

Documents required to send a child to the camp

  1 . Birth certificate of a child (or a common passport upon reaching the age of 14) -   original   (for a train)   and a copy   (for a camp).

2 . Compulsory Health Insurance Policy ( copy ). &Nbsp;    

3.   Vaccination certificate or statement of information   on carrying out preventive vaccinations, certified by the signature of the health worker and the seal of the medical institution (copy).  

4.   A certificate of the epidemiological environment (lack of contact with patients with infectious diseases) is taken at the SES at least 1 day before departure (original).

5.   Medical certificate form 079 / у from the district pediatrician, certified by the doctor’s signature and the seal of the medical institution (original). If a child travels from one camp to another, a medical certificate is taken in 2 copies (original). &Nbsp; If the child travels to another camp after, then two (2) certificates of the original are taken.

6.   Informed parental consent for medical intervention (original). The form is attached.  

7.   An explanatory note from parents written in free form on the child’s presence in the present or past of any allergic or other reactions to the administration or use of drugs, intolerance to certain products and features of child psychology, which are worth paying attention to.   ( QUESTIONNAIRE )        

8 . A certificate from the school   ORIGINAL   confirming that the child is a student (a certificate is required only for children from 10 years old).
