  1. LIMITED PASSPORT + PASSPORT COPY (all completed pages, including the first page of the passport, visas and seals, even if they are poorly viewed). The validity of the passport must be at least 3 months after the end of the visa. The passport must also contain at least 2 blank pages.

  2. TWO   SINGLE PHOTOS.   Photos should be color, crisp, matte on a white background with no corners and ovals 3.5 X 4.5 cm in size, no more than 6 months before the application for a visa, the size of the head should occupy 70% of the photograph. Scanned copies are not accepted.

  3. QUESTIONNAIRE.   The application must be   signed.   In the case of a visa for a child under 18, the application must be signed by both parents. must coincide with the signature in the passport.

  4. HELP FROM THE SCHOOL, if the child is studying.   The certificate from the institution must be signed by the official and stamped, and must be issued no earlier than one month for the time of the request.   The help should contain the following information:

-   date of issue;

-   the name of the school;

-   Student's name;

-   The full address and phone number of the school.

5.   SPONSORSHIP LETTER OF THE GUARANTEE OF COVERING CHILD FOR THE CHILD BY TRAVEL.   Only the closest relative can provide the sponsor with the documents proving the relationship.   The letter is written in any form, it must be dated and signed by the sponsor.

6.   SPEECH FROM THE SPONSOR'S WORK   with the date of entry, job and salary. The certificate of employment must be issued on the company letterhead with the seal and signature of the official, and must be issued not earlier than one month at the time of the request.

7. EVIDENCE OF PAYMENT SPONSOR   for the period of your stay in Greece. Proof of this can serve as an extract from the bank about the availability of funds on the account or a credit card statement with a copy of the credit card   for at least the three previous months. The statement must be issued on the bank's letterhead, signed by an official and certified by the bank's seal. This document must be issued no earlier than one month at the time of the request.

8.   COPY OF THE CERTIFICATE OF THE CHILD'S BIRTH.

9. In the event that the child travels alone or accompanied by third parties, you must provide the original and a copy of the notarized consent for departure from both parents.   Consent is granted from one parent only if   < / b> if the parent has an exclusive parental right (ie if the other parent has died or is deprived of parental rights), a death certificate or court order is required in these cases.

11. Copies of Russian Parent Passports   (pages with biographical data, with notes on the passport / passports issued to him / her, his / her marital status and registration in Russia).

12. COPY OF THE FIRST PAGE OF THE CONTRIBUTING PASSENGER SUPPORTING   (provided in case of escort).

13. COPY OF A SHENGEN VISA ACCOMPANYING ACCOUNT   (in case of a child's departure accompanied by an existing visa).

14. COPY OF THE RUSSIAN PASSPORT SUPPORTING   (pages with biographical data, with notes on the passport / passports issued to him / her, his / her marital status and registration in Russia). Provided in case of accompanying the child by a third person.

15. POLICY OF HEALTH INSURANCE   for travel dates operating in the territory of   SCHENGEN   with a cover amount of at least 30 € 000 + COPY.

16. CONFIRMATION OF HOTEL RESERVATION or VOUCHER   for the entire stay in Greece with a 100% payment or prepayment of accommodation   in the amount of at least 30% of the total cost of the stay (in case of booking several hotels, you must provide a confirmation of payment in the amount of not less than 30% of the cost of living each hotel.)   The confirmation or voucher should contain the following information:

- surname and name of each applicant;

- reservation confirmation number;

- the date of entry;

- departure date;

- name, address, phone and fax number of the hotel;

- confirmation of payment or prepayment of accommodation.

17.   CONFIRMATION OF TICKET BOOKING   back and forth.

18.   PREVIOUS LIMITED PASSPORT,   if it contained Schengen visas for the last 3 years + copies of these visas.

19.   COPY OF THE RUSSIAN PASSPORT   (pages with biographical data of the applicant, with notes on the passport / passports issued to him / her, his / her marital status and registration in Russia). It is granted if the child has reached the age of 14.  

20.   Statement   to the name of the camp director   indicating the length of stay in the camp.

21. A parent's completed   My Child's application issued by a travel agency (the information of the questionnaire is required by the counselor and the doctor and is confidential information that is not to be disclosed).

22.   Rules of Conduct.

23.     “ Help of the student leaving for the children's camp ”   (medical form U-79) with a note about vaccinations, chronic illnesses and recreation in the camp and southern conditions climate (it is made out at the school nurse or in a polyclinic on a residence).

24.   Help for missing contacts   with infectious patients no earlier than three days before departure.
