В том, случае, если прививки не отражены в справке по форме 079, необходимо предоставить копию прививочного сертификата по форме 063 результаты анализов: соскоб на энтеробиоз, яйца-глист и простейшие. (Результаты могут быть вписаны в форму 079) заключение дерматолога (возможно вписать в справку 079 фразу: кожные покровы и слизистые чистые) оригинал + копию свидетельства о рождении/паспорта копия медицинского страхового полиса — ОМС/ДМС. Спортивная страховка по желанию родителей. Организация рекомендует оформить на год спортивную страховку от несчастных случаев справку об инфекционных контактах и отсутствии карантина (по квартире - берется в детской поликлинике).
