To check-in and Board the train, you must have

  • the original birth certificate a child under 14 years of age, or the original passport if Your child is 14 years of age or older.

  • The inquiry under the form № 076/u-04 (health card for health vouchers) or medical certificate form 079-U (for recreational trips);
  • The conclusion of the dermatologist about the absence of infectious diseases of the skin;
  • Sanitary-Epidemiological Medical Certificate (taken 3 days before the start of the session).

  • Оригинал свидетельства о рождении или паспорта от 14 лет + Копия свидетельства о рождении для детей до 14 лет, для детей от 14 лет копия паспорта (разворот первой страницы с фотографией и страницы с регистрацией);
  • Копия полиса ОМС (с двух сторон);
  • Медицинская справка (форма 079/у) - путевка без лечения (карта действительна 2 мес); Санаторно-курортная карта (форма № 076/у) - путевка с лечением (карта действительна 2 мес);
  • Справки о прививках (в случае отказа от проведения профилактических прививок предоставить копию бланка Отказа, заверенную в детской поликлинике; в случае отсутствия сроком более 1 (одного) года пробы Манту или Диаскин-теста, туберкулинодиагностики - заключение врача-фтизиатра об отсутствии заболевания);
  • Заключение врача-дерматолога об отсутствии заразных заболеваний кожи (действительно 7 дн) ;
  • Справку об отсутствии контакта с инфекционными больными со всех мест пребывания ребёнка за последние три недели (21 день), в том числе об отсутствии контакта с вероятными больными новой коронавирусной инфекцией covid-19 в срок не менее 14 дней, выданную за 3 дня до отъезда.
  • Для детей от 15 лет необходима отметка флюорографии за последний год;
  • Справка с указанием результатов анализа на яйца гельминтов и энтеробиоз для детей всех возрастов (справка действительна 10 дн);
  • Письменное согласие клиента на виды медицинских вмешательств, включенные в перечень определенных видов вмешательств (ч.9 ст. 20 ФЗ от 21.11.2011г. № 323-ФЗ «Об основах охраны здоровья граждан в РФ») и обработку, передачу и хранение персональных данных.
