  9. Original reference from the work of the sponsor indicating the position, monthly salary and the date of employment
  10. Fin. guarantees of the sponsor - confirmation of the availability of funds (an extract from the bank at a rate of 70 euros per day).

  11. Notarized consent for departure from both parents; (… Czech Republic and other Schengen countries …) from both parents in the name of the provider
