  • результаты анализов на яйца глист и энтеробиоз;
  • подписанное согласие родителей на оказание медицинских услуг; 
  • согласие на обработку персональных данных;
  • заполненная анкета от тех, кто зарегистрирован не в Крыму;
  • анкетные данные ребенка; 
  • дополнительные сведения, необходимые для организации пребывания ребенка в лагере
  • отрицательный тест на COVID-19 (методом ПЦР) на ребенка, полученный не ранее, чем за 72 часа до заезда в Лагерь.
