When ordering a voucher, you should have:

  • completed questionnaire for child  
  • copy of the child's birth certificate / passport (id) (depending on age)

  • Consent to medical examination

You must prepare for departure:

  • medical certificate F 079 / U (certificate of the general health of the child with the statement of vaccinations from the medical card and a note on the absence of pediculosis.Takes from the doctor at the school or in the polyclinic at the place of residence.) Valid for 6 months.
  • medical certificate of absence of contact with infectious patients (taken three days before departure)
  • certificate of permission to visit the pool (if the child goes to the pool, additional payment). Valid for 6 months.
  • certificate of vaccination against encephalitis, if the participant is vaccinated (we recommend to vaccinate)

  • Копия страхового медицинского полиса обязательного медицинского страхования
  • Информированное добровольное согласие на медицинское вмешательство
  • Медицинская справка о состоянии здоровья ребенка, отъезжающего в организацию отдыха детей и их оздоровления формы № 0/79у
  • Справка об отсутствии контактов с инфекционными больными. С отметкой об отсутствии педикулеза, чесотки и контакта с ковид-больными. Срок действия 3 дня
  • Сведения о проведенных профилактических прививках. При отсутствии прививок, необходимо предоставить копию документа об отказе и причину отказа по месту жительства
  • Справка для бассейна, содержащая анализ на я/глист и соскоб на энтеробиоз
  • При отсутствии прививок на корь. АДС (дифтерия), необходимо сдать кровь на антитела иммуноглобулина кори, дифтерии
  • Согласие на пребывание ребенка в детском тематическом лагере
  • Справка о прививке против энцефалита, если участник привит. Прививка носит рекомендательный характер

Внимание!

  • Просьба не забывать справки и документы на отправку ребенка на программу, так как могут возникнуть сложности с приемом ребенка в лагере.
