  • Подписанное Согласие родителей и/или иных законных представителей Участника ТС на обработку своих персональных данных и персональных данных Участника ТС и получение рекламы;
  • Подписанное родителями и/или иными законными представителями Участника ТС Согласие на осуществление фото/видеосъемки, обнародование и дальнейшее использование фото/видео материалов;
  • Копии свидетельства о рождении/копия паспорта Участника ТС;
  • Копия полиса обязательного медицинского страхования Участника ТС;
  • Добровольное согласие на госпитализацию в случае необходимости (только для варианта с проживанием);
  • Оформленная доверенность на право передать Участника ТС Сотруднику ТС и Сотрудником ТС по окончании ТС, с правом подписи все необходимых документов от имени родителя/законного представителя Участника ТС в случае, если Участник ТС прибывает и/или покидает ТС с лицом, не являющиеся его родителем/законным представителем (только для варианта с проживанием).

Медицинские справки:

  • Допуск к занятиям по футболу на весь период, в течение которого Участник ТС будет посещать тренировки;
  • Справка о санитарно-эпидемиологическом окружении;
  • Медицинская справка «Для ребенка, отъезжающего в детский оздоровительный лагерь» по форме 079/У (только для варианта с проживанием).
