  • photocopy of the child’s birth certificate / passport (depending on age)

  • original birth certificate (up to 14 years old) or passport (after 14 years old) of a child (depending on which copy of which document was provided when issuing the ticket)
  • medical certificate Ф 079 / У (certificate on the general health of the child with an extract on vaccinations from the medical record and a note on the absence of pediculosis. It is taken from the doctor at school or in the clinic at the place of residence. Valid for 6 months. The doctor can issue a standard help form or fill out our form, which we recommend.   • certificate from the school stating that the student (for participants over 14 years old)
  • certificate of vaccination against encephalitis, if the participant is vaccinated.   Vaccination   is recommendatory.
