  • Ксерокопия свидетельства о рождении или паспорта, если ребенку уже исполнилось 14 лет.
  • Ксерокопия страхового медицинского полиса обязательного медицинского страхования.
  • Медицинская справка Ф 079/У (справка об общем состоянии здоровья ребенка с выпиской о прививках из медицинской карты). Берется у врача в школе или в поликлинике по месту жительства. Действительна в течение 3 месяцев, содержит сведения о проведенных профилактических прививках. При отсутствии прививок, необходимо предоставить копию документа об отказе и причину отказа по месту жительства с подписью и печатью врача ЛПУ.
  • Медицинская справка об отсутствии контактов с инфекционными больными с отметкой об отсутствии педикулеза, чесотки и контакта с ковид-больными (берется не ранее, чем за три дня до отъезда).
  • Информированное добровольное согласие на медицинское вмешательство (подписанный оригинал).
  • Справка о прививке против энцефалита, если участник привит. Прививка носит рекомендательный характер
