For the child it is necessary to provide to camp "Vodoley" until the conclusion of the contract and payment:

  • A copy of the child's birth certificate (for children from 14 years - passport);
  • A copy of the child's medical insurance policy.
  • Completed and signed by one of the parents application form.
  • Photo of the child 3*4 (1 item) for the application form
  • Passport of one of the parents for signing the contract, as well as a photocopy (first page and registration page).

Attendant should provide to "Vodoley":

  • A copy of the attendant's passport
  • Photocopy of the attendant's medical insurance policy
  • Exit permit for the child in the name of the attendant, iin case the attendant is not the parent of the child

Parents are obliged to ensure that the child has the following medical certificates on the day of arrival:

  • Medical certificate (079-U form) with an extract from the vaccination card with a mark on the absence of pediculosis;
  • Sanitary-Epidemiological Medical Certificate (taken 3 days before departure);

Родители должны обеспечить наличие у ребенка в день заезда следующих справок:

  • Справка по форме 079-У с выпиской из карты прививок
  • Справка об отсутствии инфекционных контактов с отметкой об отсутствии педикулеза (берется за 3 дня до отъезда)

В день отъезда необходимо предоставить Согласие на ребенка на имя сопровождающего взрослого - вожатого, если сопровождающий взрослый не является родителем ребенка.
